Remember these images from our first course, fixing healthcare delivery? I want you to re-imagine free jumbo jets, crashing every other day and killing everyone on board. As you may recall, this is the number of patients who die each year in the United States as a consequence of preventable medical errors. That's right, a 100,000 to 440,000 die each year, and statistics in other countries reveal equally alarming outcomes. If you are unlucky enough to be hospitalized in any hospital in the world, you have a 1 in 10 chance of suffering a medical error. I am sure you are thinking, why are there so many errors? The primary reason: fragmented and error prone systems of healthcare delivery. It's like working in a maze. After studying the problem of medical errors for over 30 years, I've concluded that our patchwork health systems need a major makeover. I am Dr. Fred Southwick, professor of medicine at the University of Florida College of Medicine and director of patient care quality and safety for the Division of Hospital's Medicine.
